REQUIREMENTS FOR HEARING (CPC)

REQUIREMENTS FOR HEARING:

1. Proof of Publication of the Notice of Hearing (you can claim the original copy at the office if its available)

2. Affidavit of Application & Attestation Clause.

    -  The operator should fill out this form with signature then notarized.

    - Please produce 3 copies

3. Proof of Filipino Citizenship of the owner

    - PSA Birth Certificate/Voter's ID/Philippine Passport

4. Government ID of the owner

5. BANK CERTIFICATE (Proof of Financial Capability)

    - Provide Bank Certificate with deposit amount of Php 20,000.00

    - Must be addressed to the LTFRB Main East Ave.

    - Purpose of the certificate is for requirements for issuance of Certificate of Public Convenience for             TNVS

6. Transportation Business Name issued by DTI

    - You can apply at DTI office or online with this link: https://bnrs.dti.gov.ph/

    - DTI coverage should be "NATIONAL"

    - Business must have the word "TRANSPORT" ex. "JUAN DELA CRUZ TRANSPORT"

7. BIR 2303

    - The Line of Business should be "TNVS/TAXICABS OPERATION"

    - Make sure to file the BIR 2303 on the same month of filing the DTI to avoid penalty.

    - Requirements for BIR 2303

        - TIN (Tax Identification Number)

        - BIR Form 1901

        - 1 Valid ID with current address.

        - DTI Certificate

8. ORIGINAL LTO OR/CR with year model

    - If original is not available, Certificate of Bank/Financial Institution stating that the original copy is in their possession.

    - OR Certified True Copy of Certificate of Registration of vehicle

9. 5R Picture of the Unit in Photopaper

    - Should be printed in photopaper

    - Sample Photos:

10. PROOF OF GARAGE

    - OCT/TCT  and/or Tax Declaration of the property in the name of operator.

    - If property is not owned, attach the Notarized Contract of Lease or Authority to Use indicating the title, area and duration. Then attached the ID of the owner of property with 3 signatures.

    - For Condo, Condominium Certificate of Title given that the details of the garage is indicated. 

    - LGU Zoning Certificate if you have 3 units

    - Note: Garage should be inside the MUCEP Area

11. Google Map that pinpoint the exact location of the garage. 

    -Indicate all the address of the garage.

12. 5R PICTURE OF THE GARAGE in PHOTOPAPER

    - Picture of the actual garage while the vehicle is parked.

13. Notarized Operators Data Sheet with 2x2 picture of owner

    - Provide 3 copies with original notary

    - Dimension of the Garage

    - Attached the Professional Drivers License, NBI Clearance and Police Clearance of the Driver.

14. Special Power of Attorney

15. Passenger Insurance

    - The original copy will be issued at the office

16. Certificate of Authenticity and Registration 

    - Will be provided at the office after completing the above requirements.
