HOW TO APPLY FOR NEW CPC/PA for TNVS

Steps for Applying for New CPC

2. Prepare the requirements for Application for PA

    Requirements:

        1. 4 Copies of CPC Application Form

             - We will provide the form at office and the owner of the vehicle will have to sign it.

        2. Photocopy of LTO OR/CR (if not available Sales Invoice)

            - We need the clear copy of the documents and all letter and numbers should be readable.

            - The minimum required year model of the vehicle is at least 2022.

            - At least Sedan and SUVs (Hatchback is not allowed)

        3. If the Unit is Leased/Encumbered, the owner should execute an Affidavit of Undertaking

        4. Proof of Filipino Citizenship (any of these)

            - PSA Birth Certificate

            - Voter's ID

            - Philippine Passport

            - Married Certificate

        5. Valid Government ID

        6. Special Power of Attorney

            - We will provide the form

4. We will line up your application when the slots are available at the LTFRB. Please understand that we are implementing on a first come first serve basis.
